Understanding Your Membership Terms
To maintain the benefits of your plan, please review the following administrative and payment guidelines:
- Payment & Automatic Billing. By enrolling, you authorize Avenue Dental Care-Spokane Valley to automatically process your membership dues using your preferred Visa, MasterCard, Discover, or American Express card on the scheduled dates.
- Renewal & Termination. Your membership automatically renews annually to ensure uninterrupted coverage. If you decide to cancel, we require a 30-day written notice prior to your renewal date.
- Failed Transactions. If a payment cannot be processed, you are responsible for providing an alternative payment method. Please note that a $35 fee applies to late or insufficient fund transactions, and we may re-attempt the withdrawal on a different business day.
- Contractual Commitment. If the membership is canceled or defaults during the fiscal year, the remaining balance of the annual contract remains due in full.
- Updates to Your Account. To avoid payment interruptions, please notify us at least 15 days before a scheduled withdrawal regarding any changes to your card status, such as new expiration dates or closed accounts.
- Fee Adjustments. While we strive for consistency, we reserve the right to update treatment prices. New fees apply immediately to all future procedures, and previous estimates from non-scheduled treatment plans will be superseded by current rates.
