These services will be discounted from our usual and customary rates, providing substantial savings.<\/p>\n<h2>Membership Terms and Conditions<\/h2>\n<p>By enrolling, you agree to the following important terms regarding payment and renewal:<img decoding=\"async\" class=\"alignright wp-image-16851 \" src=\"https:\/\/www.avenuedentalcares.com\/spokane-wa\/wp-content\/uploads\/sites\/3\/2026\/02\/insurance_financing_spokane_wa_dentist-1.jpg\" alt=\"\" width=\"400\" height=\"469\" \/><\/p>\n<ul>\n<li><strong>Payment Authorization.<\/strong> You authorize Avenue Dental Group to automatically debit your payment from your designated Visa, MasterCard, Discover, or Amex Card as scheduled.<\/li>\n<li><strong>Failed Payments.<\/strong> If Avenue Dental Group is unable to process your scheduled payment, you are responsible for making an alternate payment arrangement. A late fee and\/or an insufficient fund fee of $35 per transaction will apply. We are authorized to withdraw your payment on a different day if the initial attempt fails.<\/li>\n<li><strong>Contract Cancellation.<\/strong> If the membership contract is canceled or defaults for any reason within the fiscal year period, the remaining month(s) of the contract will still be due in full.<\/li>\n<li><strong>Automatic Renewal.<\/strong> The enrollment period will automatically renew each year for an additional year without separate notice.<\/li>\n<li><strong>Termination Notice.<\/strong> If you wish to terminate the contract, a 30-day written notice is required before the renewal date to prevent the automatic renewal for the following year.<\/li>\n<li><strong>Card Changes.<\/strong> You must notify our office fifteen (15) days prior to a scheduled withdrawal of any changes made to your designated credit\/debit card, including but not limited to closed status, account changes, or changes in the card's expiration date.<\/li>\n<li><strong>Fee Changes.<\/strong> Avenue Dental Group reserves the right to increase treatment prices at any time.
